HUI Format | Which HUI questionnaire format is right for your needs? The HUI health status classification system can be interviewer or self-administered, proxy or self-assessed, with a range of 'standard' recall periods to suit most surveys. This page provides 'at-a-glance' details of the different versions available including the new HUI Web-based administration application. |

Application Form | On-line application form to submit a profile of information related to your study proposal. It allows the HUInc service personnel to review your proposal and advise which HUI instrument is best suited to your needs. |

Web-based System *NEW* | Brief description of the new (2005) HUI web-based administration application system. The HUI can now be administered via the internet. Results, including questionnaire responses plus all HUI2 & HUI3 variables and utility scores, are emailed direct to the study coordinator as soon as a respondent completes the questionnaire. |

George Torrance - recipient of this year's "Avedis Donabedian Outcomes Research Lifetime Achievement Award" awarded at the ISPOR 11th Annual International Meeting in Philadelphia, May 2006. George is co-developer of the HUI. The award is named in honor of Avedis Donabedian, one of the pioneers in the measurement of patient outcomes and quality of care. Previous winners include John Ware and Michael Drummond. | |
